Sponsor & TC Forms

This page of quick links to commonly used OSP and Sponsor forms is provided for your convenience. If you would like us to include a sponsor who isn't here, please email the director. Also, let us know if a link doesn't work. Chances are it's because the sponsor has moved the link. We'll investigate and update our page.

Please note that most of our major federal sponsors (except NSF and certain US Ed programs) now use Grants.gov for proposal submission. Please see the Grants.gov link to the right for complete information. Grants.gov does not require a user name or password in order to use the system. However, the NSF fastlane system and the NIH Commons(used for proposal tracking and review and non-competing grant applications) do require user names and passwords. Please contact Cristy Ettenson at x 4106 to set up access.

Private/Foundation Sponsors: note--many private sponsors do not have special forms; links are to application instructions.

For links to other private sponsors, use the Foundation Center's Directory of Grantmaker Websites.

State/Local Sponsors:

New York State Department of Education choose the appropriate division, then choose "request for proposals"; application forms are included in program descriptions.

New York City Department of Education choose appropriate RFP and use forms included with it.

Local Private Foundations Common Report Form (CRF) 60 Private Foundations that fund locally in the tristate area have adopted the CRF to assist in multiple applications for the same project. A list of those foundations is provided at the link above.
