Application
and Service Procedures
Prospective clients are required to fill out an application for
services. Applications may be obtained online, by telephone, or
in person at the Center.
Individuals seeking couples or domestic partnership counseling are
encouraged to fill out separate applications.
To obtain an application, see below to download forms. You may
also call the Center, Monday through Friday, between 9 a.m. and
5 p.m. at (212) 678-3262 to have an application mailed to you.
To apply, please print the appropriate application (adult or child/adolescent) and consent form. Applications
cannot be filled out online.

Applications may be submitted in person, by mail, or you may contact the Center to discuss any further options.
When the completed applications are received, a staff member will
contact you to make an appointment. If you want to find out the status
of your application at any time, you can call the Center directly.
Incomplete applications will not be processed.
Note: We are a training facility, and we may not be a suitable
agency for some cases with a specific psychological and/or psychiatric
history. In this event, we will inform the applicant immediately
and offer an appropriate alternate resource.
